Responsibilities
* Maintain a caseload of 15-20 patients and perform casework responsibilities:
* Perform intake interviews with patients to evaluate appropriateness for treatment and compile all information required to process the patient’s request for service.
* Write complete and accurate Intake Bio-Psycho-Social assessments within the required timeframe, including the patient’s history and an assessment of dependencies.
* Establish collaborative relationships with patients in a supportive atmosphere to assist them in confronting drug-related behaviors and achieving sobriety.
* Use age-appropriate interventions with patients.
* Meet with patients as noted in the treatment plan.
* Provide individual, group, and family counseling.
* Educate patients on all areas of substance abuse and recovery.
Complete clinical/counseling records as required, in accordance with The Guidance Center’s policies and procedures, and in compliance with all relevant rules and regulations.
Develop multi-disciplinary treatment plans that address individualized patient needs with realistic goals and objectives within the required timeframe.
Ensure progress notes reflect problems, goals, objectives, and interventions as described in the treatment plan; notes should be accurate and completed within the designated time period.
Complete Discharge Summaries within the required timeframe.
Knowledge of Laws and Policies
Demonstrate thorough knowledge of State & Federal laws, clinic policies, and procedures:
* Stay updated on changes in agency and CDTC policies and procedures.
* Interpret clinic policies and regulations to patients and assist them in adhering to these guidelines.
* Maintain patient confidentiality in accordance with CFR Part 42.
Coordination and Teamwork
Coordinate patient care with all treatment team members:
* Participate in Interdisciplinary Team meetings as a team member.
* Act as a liaison to community agencies and governmental organizations.
* Make appropriate referrals to outside agencies based on patient needs.
* Attend weekly group and individual supervision.
* Support team decisions regarding patient care and follow through responsibly.
* Maintain a positive attitude during conflict resolution.
* Understand issues related to varying age and gender populations served.
* Assess crisis situations and report observations appropriately.
* Identify signs of child abuse/neglect and domestic violence promptly and report to supervisors.
