Townend house will accommodate up to 24 residents, offering high-quality facilities and focuses on a person-centred approach to support individuals on their recovery journey. This organisation is committed to safeguarding and promoting the welfare of vulnerable adults.
Overview:
We are seeking a compassionate and proactive Outreach Manager to lead our engagement efforts in support of individuals recovering from drug and alcohol dependency. Based at our residential rehabilitation house, you will play a vital role in connecting residents to community resources, strengthening referral pathways, and promoting long-term recovery. This role blends strategic outreach, advocacy, and hands-on support to ensure every individual has the opportunity to rebuild their life with dignity and purpose.
Key Responsibilities
Community Engagement & Referral Pathways
* Build and maintain relationships with addiction services, healthcare providers, social workers, and local authorities
* Represent the service at community forums, recovery networks, and stakeholder meetings
* Support treatment and / or intervention as required to the service treatment contract.
Resident Support & Aftercare
* Support residents in planning for life after rehab, including housing, employment, and continued recovery services
* Support to develop recovery plans
* Facilitate connections to peer support groups, counselling, and harm reduction services
* Planning, liaison, communication and support to individuals to continue in their recovery following discharge
* Support following unplanned discharge from rehabilitation to minimise risk and support individuals to continue their recovery
* Organise, develop and plan community events and group activities
Monitoring & Reporting
* Track outreach activities, and community engagement metrics
* Contribute to service development through feedback, data analysis, monthly audits and lived experience insights
Person Specification
Essential
* Experience in outreach, case management, or addiction recovery services
* Strong understanding of substance use, trauma-informed care, and recovery models
* Excellent communication and interpersonal skills to develop professional relationships, organisational skills
* Ability to demonstrate empathy, care and compassion
* Ability to work independently and collaboratively across sectors
* Knowledge of Scottish health and social care systems, including ADPs (Alcohol and Drug Partnerships)
* Registered with the Scottish Social Services Council (SSSC) or eligible for registration upon appointment
Desirable
* Lived experience of recovery or working in residential rehabilitation
* Familiarity with safeguarding, risk assessment, and crisis intervention
* Experience of community engagement and inclusion legislation
