EHCP Coordinator - Medway
Position: Permanent, Full-time
Salary Range: £31,283 - £37,181
This is a hybrid role requiring attendance at our Office and flexible work across our Hubs. We seek candidates with experience in both pre and post 14 EHCP plans and previous EHCP management experience.
We are looking for dedicated and experienced Education, Health and Care (EHC) Coordinators to join our SEND and Inclusion Service. The successful candidates will coordinate and manage EHC plans and annual reviews for young people aged 0-25 with special educational needs and disabilities.
This role involves working closely with young people, their families/carers, schools, further education colleges, training providers, and relevant professionals to develop high-quality, outcome-focused EHC Plans.
Key Responsibilities:
1. Oversee the entire process of EHCP maintenance, reviews, and amendments, ensuring compliance with statutory deadlines.
2. Collaborate with families, schools, health, and social care professionals to tailor EHCPs to meet individual needs.
3. Ensure all EHCPs are developed and maintained according to the Children and Families Act 2014 and the SEND Code of Practice (2015).
4. Act as a primary contact for families, providing guidance and support, and ensuring a fair, transparent process.
5. Maintain accurate records, track progress, and contribute to service development by identifying trends and areas for improvement.
