Role Overview
We are looking for an experienced Care Manager to oversee the coordination and delivery of complex care packages, ensuring all clients receive safe, effective, and person-centred care. This role involves managing staff allocation, maintaining compliance, and ensuring high standards of service delivery.
Key Responsibilities
* Manage and coordinate day-to-day operations of complex care packages
* Ensure all shifts are safely staffed with suitable and competent personnel
* Work closely with recruitment to ensure effective staffing pipelines
* Maintain strong relationships with:
* Clients and families
* Case managers
* Healthcare professionals
* Monitor and ensure compliance with:
* CQC standards
* Company policies
* Safeguarding procedures
* Conduct regular reviews of care packages and staff performance
Essential Requirements
* Experience in a care coordination or care management role
* Strong understanding of complex care packages
Knowledge of:
* Manual handling
* Safeguarding
* Risk assessments
* Excellent organisational and time management skills
* Ability to work in a fast-paced environment
* Strong communication and problem-solving skills
Desirable
* NVQ Level 3 or above in Health & Social Care
* Experience working within a complex care or domiciliary care agency
* Understanding of rota systems and workforce planning
