Responsibilities
* Oversee and undertake physical health checks and provide health promotion support to complex patients with severe and enduring mental health problems under the care of low secure forensic services and the community forensic team.
* Provide advice and support clinical and social care interventions for service users while holding a caseload of community forensic patients.
* Supervise and support teams to deliver high standards of physical care to patients with long-term conditions, ensuring up-to-date, evidence-based practice and providing leadership to nursing teams.
* Lead on preparation and delivery of health promotion initiatives, ensuring materials are up to date, available in various languages, and accessible across services.
* Support administration and facilitation of service user forums and feedback related to physical health and health promotion initiatives.
Main Responsibilities – Detailed
* Advise and contribute to the delivery of high-quality nursing care for patients with physical health issues within the forensic service.
* Lead development and delivery of nursing care plans, advising on physical healthcare aspects including diet, weight management, exercise, lifestyle, sexual health, smoking, and monitoring of medication side effects.
* Develop, coordinate and contribute to physical health and well-being training programmes and educational materials for staff in collaboration with senior service leaders.
* Be conversant with Adult Safeguarding, NSF, Mental Health Act, Mental Capacity Act, and key national Mental Health policies.
* Provide role modeling of evidence-based practice, collaborate with colleagues, and monitor care effectiveness within the service.
* Participate in audits and research related to physical wellbeing of patients throughout the service.
* Work with senior personnel to ensure systems monitor and record physical health and wellbeing across services.
* Provide clear clinical leadership through coaching, modelling, training and clinical teaching.
* Undertake clinical work and continuous professional development to maintain practice in line with the NMC Code and Mental Health Legislation.
* Contribute to complex case discussions and investigations of serious incidents and complaints.
* Provide data and reports on physical health care standards within the service.
* Participate in nursing assessment, care planning, discharge planning, and keep clinical records to high standards.
* Collaborate with external care partners (GP, CMHT, Addictions services) for after-care planning.
* Work with relatives and carers providing guidance and support where possible.
* Facilitate and co-facilitate healthcare clinics or therapeutic groups with session planning and evaluation.
* Supervise and administer medications as required, in line with storage and administration policies; coordinate with pharmacy as needed.
* Participate in governance, policy adherence, and adherence to clinical standards.
* Contribute to risk assessments and maintain appropriate observations as part of patient care.
* Develop, implement and evaluate care programmes using a problem-solving approach to meet patient needs throughout admission and discharge planning.
* Engage in supervision and appraisal processes for self and staff; attend meetings as requested.
Qualifications and Experience
Education and Qualifications
Essential:
* Registered General Nurse
* Mental Health Nursing qualification (RGN) with RNLD or RMN
* Evidence of continuing professional development
* Active NMC registration
Desirable:
* District Nursing / Community / Practice Nursing experience
* Post-Registration Leadership Training or qualification
Previous Experience
Essential:
* Experience in senior nursing positions in healthcare services for adults
* Experience with safeguarding legislation
Desirable:
* Preparation of reports/presentations to internal/external agencies
