Clinical Dementia Lead/ Clinical Specialist Practitioner
The Peter Sowerby Dementia Breakthrough Partnership was formed in 2019 to realise the full potential of GPs to transform dementia support; to improve the lives of millions of people with dementia, today and tomorrow, through primary care-led post-diagnostic support.
Alzheimer's Society were very fortunate to be awarded funding from the Peter Sowerby Dementia Transformation Fund with the aim to transform the quality of life of people affected by dementia by empoweringprimary care and community care providers to deliver high-quality support at the point of, and following, diagnosis.
In partnership with NSFT, we are aiming to deliver on the initiative by offering this post to a suitable applicant. We are looking for a Mental Health or Learning Disabilities Nurse who has suitable experience and enthusiasm for working with people who have memory and cognitive problems and who is passionate about improving the lives of the people who do. * Review MCI patients and in doing so, help to embed this into the culture of Primary Care.
