* Managing and scheduling staff rotas to ensure consistent and effective care delivery across the service.
* Responding to last-minute care changes or emergencies to minimise disruption to service users.
* Communicating regularly and professionally with clients and their families to ensure their care needs are being met and any concerns are promptly addressed.
* Liaising with pharmacies to ensure timely and accurate delivery of medication for service users.
* Supporting the Registered Manager with the completion of client assessments and risk assessments to maintain safe and person-centred care planning.
* Building strong relationships with care staff to promote high standards of care.
* Ensuring all documentation and records are kept up to date and in line with CQC regulations and internal policies.
* Experience working in a Care Coordinator role within domiciliary care for a minimum of 12 months.
* UK Driving Licence and use of own car.
