Provide specialist spinal services at our world renowned National Spinal Injuries Centre at Stoke Mandeville Hospital, the birthplace of the Paralympics
Nationally recognised for urology and skin cancer services
Regional specialist centre for burns, plastic surgery, dermatology, stroke and cardiac services
Deliver community services in health centres, schools, patients' own homes, community hospitals and community hubs.

The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission of patients into Hospital and facilitate timely discharge into the community.
The team is committed to ongoing education on the job and development opportunities are available within the organisation. Successful applicants who don't already have a Level 3 Qualification in Health and Social Care will be expected to work towards that in post.
Providing general and specific rehabilitation focused care to adults, including personal care, exercise supervision, rehabilitation with domestic tasks and equipment delivery within competencies
Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of nursing, social and rehabilitative care to patients, working in collaboration with senior staff.
Improve the care of patients in the community, so patients can remain safely in their own home for as long as possible
Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
Willingness to further develop your clinical expertise in a range of areas A full UK driving licence and access to a car with Business insurance for work purposes

Education, Qualifications & Training



Health and Social Care Level 2 or 3 Diploma or equivalent relevant patient facing experience and willingness to undertake Level 3 qualification.
Have the ability to undertake training to acquire the skills appropriate for development in the post
Previous caring experience of working in Health or Social Care environment.
Awareness of the needs of older people

Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.
