Pathway Lead - Older Adults Outreach Mental Health Team (Band 7)
Location: South Staffordshire West/East locality, Midlands Partnership NHS Foundation Trust.

The Outreach Team provides a 7‑day service (08:00‑18:00) delivering a proactive and responsive approach to supporting older adults who require intensive intervention and home treatment as an alternative to hospital admission.
This role combines clinical expertise with operational leadership responsibilities, working closely within a multidisciplinary team and alongside partners across physical health, social care, and primary care services.
Main duties of the job
- Deliver high‑quality, person‑centred care to individuals referred to the Older Adult Outreach Team.
- Provide day‑to‑day clinical and operational leadership, including supervision of staff.
- Support effective service delivery and contribute to team performance.
- Participate in Collective Leadership and Quality Improvement initiatives.
- Work collaboratively with MDT colleagues and external partners, including GPs and social care.
- Maintain accurate clinical records in line with organisational standards.
- Ensure compliance with mandatory training and take responsibility for ongoing professional development.
Person Specification
- Post‑registration experience and/or evidence of formal courses and training at post‑graduate diploma level.
Experience
- Experience of managing multi‑disciplinary teams.
- Experience of effectively initiating, developing, implementing and evaluating operational processes.
- Demonstrable experience in performance management.
- Experience in the direct line management of health or social care staff.
- Significant demonstrable post‑qualification experience at Band 6 working in a mental health / specialist inpatient setting.
- Demonstrable experience of effective multi‑disciplinary team working.
