Job Overview
We are looking to recruit a Community Support Worker for our Assertive Outreach Team. The successful applicant will deliver care and treatment for service users within the team.
The team is part of the Milton Keynes Mental Health Hub, which provides interventions such as Peer Support, Individual Placement Support, Coaching, and/or Specialist Therapy. Interventions are delivered both as groups and individual 1:1 sessions.
Main Duties of the Job
The successful applicant will work as part of a team supporting service users with complex presentations and difficulties engaging with the service. The post holder will need to have a flexible approach and a solid understanding of risk assessment, care planning and the recovery model.
Person Specification
Qualifications
* Good general education up to GCSE A level standard / NVQ level 3 or equivalent experience
Experience
* Knowledge of the Mental Health Act and Mental Capacity Act
* Experience of community-based working
* Knowledge of relevant legislation and policies
* Experience of multi-disciplinary working
* Substantial experience within a Mental Health Setting
Knowledge
* Experience of a rehabilitation approach to include an understanding of recovery and re-enablement
Personal Disposition
* Car driver with access to a fully insured vehicle fit for work purposes
