Job Title: Social Worker – Transition Team Location: Bristol Rate: £32 per hour Bristol is actively seeking a full-time Social Worker to join our dedicated Transitions Team. What will your responsibilities be? In this role, you will support our triage decision-making process, produce care plans, and work directly with individuals in Bristol who are transitioning to adulthood. You will ensure they receive the necessary support and guidance during this critical period.
