Duties and Responsibilities
Key results from the job holder
To deliver care to 24 patients in the Jean Heyes Reablement Unit primarily operating as a rehabilitation unit, preparing patients from the acute trust to regain independence before being safely discharged back into their usual home setting.
Planning and Organisational Duties
* Accountable for nursing practice and takes every opportunity to sustain and improve his/her knowledge and professional competence.
* Planning and managing care under the supervision of other members of the disciplinary team following diagnostic pathways and utilising community specialists
* Participates in the assessing, planning, implementing and evaluating programmes of care
* Monitor patients conditions, recognise deterioration and take appropriate clinical action
* Administer medications in line with policy
* Support patients to achieve their rehab goals and promote independence
* To be sensitive to the needs of patients in relation to courtesy, privacy and dignity.
* To be familiar and adhere to Trust and Divisional objectives, policies, guidelines and current legislation.
* To be competent with skills such as venepuncture, complex wound dressings, catheterisation and clinical observations
* To be able to deal with an emergency and understand the pressures and responsibilites involved
* Ensure safe discharges occur and send relevant referrals for ongoing care on discharge as appropriate
* Adhere to professional, legal and ethical standards as set by the NMC
Communications and Key Working Relationships
* Communication with all key members of the multidisciplinary team
Responsibility for Human Resources
* Follow all WWL policies and procedures
* Ensure mandatory training is up to date
Responsibility for Teaching
* Responsible for teaching any students on the Unit
* Responsible for orientating new starters

* Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Qualifications
* NMC registered Adult Nurse
* Experience within the UK setting in healthcare
* Experience in a rehab community setting
Experience
* Experience of working in a similar setting
* Experience in clinical hospital setting
* Experience of multi-disciplinary working (e.g. working closely with physio/OT)
* Experience with safe discharge planning
Skills
* Good verbal and written skills
* Wound care & Pressure area care/knowledge
* Venepuncture
* Teaching & assessing skills (including supervision of students)
Knowledge
* Experience and skills clearly demonstrating a good level of nursing knowledge
* Range of extended clinical skills & demonstrate clinical knowledge.
* Knowledge of community nursing and risks of being a nurse-led unit
