We are currently recruiting for further Lead Reablement workers part time (28 hours per week) in March, Cambridgeshire. The Reablement Service provides care at home to people aged 18 upwards. Support is delivered in an enabling way so that people can maximise their independence. Reablement support is a short term service. The Lead Reablement workers play a crucial role in supporting individuals transitioning towards independent living. They are the first point of contact for service users, working closely with Occupational Therapy teams and other health professionals to set personalised goals. These goals guide the service user through their journey until they achieve independence. Additionally, Lead Reablement workers act as key contacts, referring users to community support services and continuously monitoring their care throughout the reablement process.

The Lead Reablement Worker plays a key role in delivering short‑term, person‑centred support that helps individuals regain confidence and independence in their daily lives. This role involves working closely with service users, their families, and other professionals to create tailored reablement plans and risk assessments that promote recovery and reduce reliance on long‑term care.


Day‑to‑day responsibilities

* Assess individual needs and create tailored reablement plans focused on recovery and independence.
* Work hands‑on with service users to relearn essential daily living skills such as personal care, mobility and meal preparation.
* Monitor progress, review plans and adapt support as needed to ensure goals are met.
* Collaborate with health and social care professionals, families and other agencies to provide a holistic approach to care.
* Maintain accurate records and documentation to ensure compliance and quality standards.

This role involves travel across Cambridgeshire. You will need access to a car, a full driving licence and business insurance. You will receive excellent training and supervision as part of an established and supportive team. A car lease scheme and enhancements for weekend working are available. Your weekly hours follow a Monday‑Friday rota (typically between 08:00 and 16:30, to be confirmed at interview). The role also includes alternate weekend shifts and one late shift (12:00‑20:00) every other week.

You will need a proficient level of written and spoken English for case recording and an NVQ Level 3 or equivalent apprenticeship. Strong communication skills are essential, as will be your ability to support and liaise with many health professionals, service users and families. Experience of supporting a person with care and support needs and knowledge of The Care Act and CQC requirements are required. You should be able to manage a diverse caseload, prioritise tasks in a fast‑paced environment, and demonstrate awareness of equality, diversity and inclusion, and safety and confidentiality.
