The aim of the Health and Well-being Coach is to develop collaborative relationships and empower individuals to take control of their own health and wellbeing.
The post holder will support patients in adopting healthier lifestyles and coach individuals to achieve personalized healthcare objectives. The successful candidate will demonstrate effective team working and a strong commitment to delivering the Abbey Health Primary Care Network (PCN) agenda, providing excellent care for all patients within the PCN.
Candidates should be empathetic, reflective, self-aware, and enjoy working with diverse groups. Good communication and negotiation skills are essential, along with the ability to deliver high-quality, non-judgemental services. The role requires a proactive, flexible attitude, teamwork, and a commitment to personal development. Ongoing supervision, skills development, and support will be provided to foster continuous growth.
The successful candidate will work under the supervision of an Allied Health Professional (AHP) Clinical Lead, providing patient care at our PCN sites. The role involves delivering personalized, one-to-one, and group coaching to support individuals with long-term conditions to manage their health and wellbeing. The coach will collaborate with GPs and the multidisciplinary team (MDT) to help patients identify priorities, set meaningful goals, and build confidence in self-management. This includes promoting behavior change, supporting self-management education, peer support, and linking patients with community services.
The role also involves using IT systems and referrals to identify suitable patients, managing a caseload, and contributing to MDT meetings. The coach will share expertise in personalized care approaches across the PCN to embed coaching within services. A key focus is reducing health inequalities and supporting patients to live independently, while promoting positive risk-taking, shared decision-making, and improved health outcomes.
