Job Description:
To live in the client's home and support their daily living as assessed by Caremark. To meet the personal care and support needs of clients in a way that respects the dignity of the individual and promotes their independence. The care and support provided by Care and Support Workers is expected to include support that would reasonably be given by members of the client’s family and is not to include tasks that would normally be undertaken by a trained nurse.
Principal Responsibilities:
1. Assist clients with morning and evening routines, including dressing, undressing, washing, bathing, and toileting.
2. Help clients with mobility and physical disabilities, including incontinence and use of personal aids and equipment.
3. Care for clients who are temporarily sick, including bed nursing and feeding assistance.
4. Provide care and support for terminally ill clients.
5. Promote mental and physical activities through talking, outings, reading, writing, hobbies, and recreation.
6. Make and change beds, perform light cleaning, empty commodes, and general tidying.
7. Inspect, launder, and mend clients’ clothes.
8. Set tables and trays, prepare and serve meals, and handle cleaning up.
9. Supervise safe food preparation and storage.
10. Read and write reports and participate in client reviews as required.
11. Adhere to all Caremark policies and procedures at all times.
12. Report significant changes in a client’s health or circumstances to the Field Care Supervisor or Care Manager.
13. Encourage client independence.
14. Liaise with professionals and other agencies as required.
15. Undertake night duties as reasonably practicable.
16. Communicate effectively with other Care and Support Workers assigned to the client.
17. Provide emotional support as needed.
18. Assist with appointments and reminders.
19. Support with medication and promote clients’ independence in self-administration.
20. Liaise with clients' family or carers as directed by line management.
