Deputy Community Nurse Team Lead – Camborne Community Nursing Team

We are looking for a dynamic, passionate and motivated nurse with experience of Community Nursing to be a Team Leader within the Camborne Community Nursing Team.

The successful candidate will work with the team to ensure provision of a holistic nursing assessment and high quality, safe, evidence‑based care to patients and carers with the aim of maintaining patients in their own homes. Our patients are at the centre of all that we do, so we are looking for nurses who are calm, caring, compassionate clinicians with an interest in delivering the highest quality care and in supporting our patients to lead full and independent lives whilst promoting self‑management of their health conditions.

This will be an exciting and evolving job role within the Primary Care Network and will explore new multi‑disciplinary ways of integrated working. Therefore flexibility and a willingness to explore new exciting ways of working is a must and adaptability is required.


Main duties of the job

* As an autonomous practitioner, the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs.
* They will manage a clinical caseload within a community nursing team with guidance from the Community Nurse Team Leads, Community Matrons and Integrated Care Team Leads.
* Ensuring provision of a holistic nursing assessment and high quality, safe, evidence‑based care to patients and carers with the aim of maintaining patients in their own homes.
* This includes physical examination and detailed history taking, diagnosis of deterioration in pre‑existing conditions and treatment planning.
* Focusing on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings.
* Implementing personalised care plans and collaborating with the whole community resource to facilitate ongoing care management.
* Working in partnership with patients and carers and members of the multi‑disciplinary team to offer support and advice and promote informed choice; maximising independence and optimising quality of life.
* Providing clinical management and leadership to all staff within the team.

Person specification


Education, Qualifications and Relevant Experience

* Current professional registration – NMC
* Experience of working with patients with complex and challenging needs
* Postgraduate specialist training to degree level in a relevant subject area


Skills and aptitude

* Excellent written and verbal communication skills
* Strong commitment to teaching and learning


Knowledge and abilities

* Knowledge and application of workforce planning, supervision and appraisals
* Strong teamwork; able to motivate and engage positively with colleagues

Salary: £39,959 to £48,117 a year (Pro Rata)
