We are looking for an experienced and compassionate Community Night Nurse to join our dedicated team at Partnering Health Limited (PHL). Youll play a key role in delivering safe, patient-centred urgent care directly in peoples homes, ensuring the highest standards of care during out-of-hours service. As a valued member of our clinical and operational team, youll work independently, using your expertise to assess, treat, and support patients with a wide range of conditions. Youll provide both in-person care in the community and telephone advice, ensuring patients receive safe, effective, and timely support. Key Responsibilities Deliver holistic care in patients homes, from assessment through to treatment and medication administration. Provide safe and effective health assessments and telephone advice. Work collaboratively as part of a multi-disciplinary team, ensuring seamless, evidence-based patient care. Maintain accurate and timely records in line with NMC guidelines. What Were Looking For Essential: Registered with the NMC/HCPC. Minimum 4 years post-qualification with at least 2 years experience as a Community Nurse. Excellent communication, organisational, and decision-making skills. Ability to work independently and as part of a multi-disciplinary team. Enhanced DBS clearance and eligibility to work in the UK. Full, clean driving licence. Desirable: BSc in Nursing. Experience with Adastra and CHIE clinical IT systems. Advanced clinical training (e.g. History Taking, Physical Assessment, Clinical Decision-Making Modules). Level 3 Safeguarding (Adults & Children). Why Join PHL?
