We are seeking a dedicated Hospital In Reach Recovery Coordinator to provide a vital link between our community service and hospitals in Reading.

* Location: Reading
* Hours: 22.5 per week (Part-Time)
* Salary Range: £27,861.26 - £32,002.35 (pro rata for part-time)
* Contract: Fixed Term until 31st March 2026
Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, salary and payments are pro-rata based on contracted hours.
Responsibilities
* Work closely with A&E and other departments to develop and implement care plans for frequent hospital attenders.
* Serve as the first point of contact for hospital staff regarding our service users to ensure seamless care delivery.
* Assess patients attending or admitted to hospital with alcohol-related issues using AUDIT screening tools, providing brief interventions and referrals.
* Provide advice, support, and training to hospital staff on drug and alcohol awareness, screening, assessment, and referral pathways.
* Collaborate with community substance misuse teams for follow-up of repeat presentations and discharges.
* Inform GPs of patient status and discharge plans within 5 days of hospital discharge.
* Develop links with specialist acute services.
* Educate and support carers, contributing to their needs assessment.
* Maintain excellent understanding of drug and alcohol issues and demonstrate experience in related fields.
* Possess training and/or experience in psychosocial interventions.
* Have a sound working knowledge of safeguarding procedures.
* Work sensitively with diverse cultural issues.
* Commit to multi-agency collaboration.
