Health and Wellbeing Coach
Location: Buckinghamshire (travel/drive required). You must be able to drive and have your own transport.
Overview
We have an exciting opportunity to join the Buckinghamshire Team. The role is 37.5 hours per week, working flexibly across Monday to Saturday with evenings when required. Salary: £24,570 – £28,700.
Responsibilities
* Provide personalised health and wellbeing coaching to individuals to empower them to take control of their health and wellbeing.
* Support lifestyle behaviour change across core areas including healthy eating and nutrition, smoking cessation, physical activity, weight management, and alcohol consumption; NHS Health Checks (outreach).
* Deliver coaching face-to-face, by telephone, and digitally to individuals and groups using a mix of one-to-one and group sessions.
* Carry out triage and assessment, set agendas, support goal setting, problem solving, motivational interviewing, and signposting.
* Deliver services in line with quality standards, government guidelines, and role boundaries; identify when coaching is not appropriate and escalate as needed.
* Support outreach community engagement, including screening services, events, external meetings, and roadshows.
* Work to contract performance requirements, including SOPs, data protection, information security, Health and Safety, safeguarding; participate in audits and continuous professional development.
* Maintain accurate caseload management, data collection, and reporting to support population health and coordinated services.
Qualifications & Experience
* Relevant health coaching qualification or accredited health coaching skills programme.
* Minimum six months of direct health coaching delivery experience.
* Experience delivering behaviour change interventions for lifestyle-related issues (e.g., smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction).
* Experience supporting vulnerable individuals through a change process; caseload management with a Case Management System.
* Ability to input and process data with accuracy and meet deadlines.
* Evidence of continuous professional development and experience in health, community, or social care environments.
* Desirable: membership of a professional body (e.g., ICF, EMCC, AoC, UKHCA); degree in a relevant subject; remote/digital coaching experience; experience with long-term health conditions.
Individual Competencies
* Strong understanding of the social determinants of health and population-based approaches including segmentation and risk stratification.
* Knowledge of behaviour change principles and models (e.g., COM-B, Behaviour Change Wheel).
* Excellent verbal and written communication with diverse audiences.
* Capability to manage time, data, and caseload effectively; maintain confidentiality and safeguarding principles.
* Proficient in using digital tools and Microsoft Office; comfortable with evolving systems.
* Commitment to flexible working (including evenings and weekends) to ensure service availability for all residents.
* Commitment to personal development and delivering high-quality services aligned with KPIs and SOPs.
