<div><p>We are looking for an experienced Practice Nurse to join our friendly, supportive team for 30 hours per week</p><p>Our clinical system is EMIS Web. We use Docman, DXS and other clinical applications to support our clinical system.</p><p>We are a 5 GP Partner, 3 salaried GP training practice based in Dartford with excellent travel links to the A2/M2, M25 and A20/M20 with parking onsite.</p><h3>Main duties of the job</h3><p>The practice nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of the practice.</p><p>The range of duties include but are not limited to:</p><ul><li>Cervical cytology</li><li>Childhood immunisations</li><li>Dressings and wound care</li><li>Chronic disease monitoring</li><li>Family planning</li></ul><p>Applicants must have excellent communication skills and be able to work independently and as part of a team.</p><h3>About us</h3><p>We are a 5 GP partner and 3 salaried GP Training practice supported by a multi skilled team including clinical pharmacists, paramedics, physiotherapist and additional PCN roles.</p><p>We are part of Dartford MODEL PCN working closely with our PCN practices.</p><h3>Job responsibilities</h3><p>Job responsibilities:</p><p>Assist in and perform routine tasks related to patient care as directed by GPs</p><p>Interest in the long term management of chronic diseases</p><p>Cytology</p><p>Wound care / Ulcer care / Doppler / compression bandaging</p><p>Travel Health</p><p>Removal of Sutures</p><p>Venepuncture</p><p>Routine immunisations / Childhood immunisations</p><p>Chaperoning and assisting patients where appropriate who are being examined by another clinician</p><p>Requesting pathology tests, for example urine culture, swabs</p><p>Following agreed clinical protocols with referral to GPs and other services as appropriate</p><p>Other tasks will include</p><ul><li>Maintain general tidiness and cleanliness of nurses and treatment rooms, including stocking and rotating items as required</li><li>Maintaining and cleaning equipment used by the nurses</li><li>Participation in administrative systems in the practice</li><li>At all times there will be an need to maintain accurate records</li><li>Attend and participate in any practice meetings when required</li><li>Any other delegated duties appropriate to the post</li></ul><h3>Qualifications</h3><ul><li>Registered first level nurse</li><li>Relevant nursing/health degree, Mentor/teaching qualification.</li></ul><h3>Experience</h3><ul><li>Experience of working in Primary Care</li><li>Management of long-term conditions, wound care, cytology and child immunisations.</li><li>Involvement in implementing and using protocols and clinical guidelines</li><li>Experience of audit</li><li>Experience of working in a GP Practice.</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p></div>
