The job is STHC Payscale E, which equates to £38,032 - £45,965 per annum

Job responsibilities
Tobe responsible for the implementation of processes for the effective managementof patients with long-term conditions using evidence-based practice includingcare for elderly and housebound patients.
Workingas a senior member of the practice multidisciplinary team, the post holder willensure nursing services are delivered effectively to the entitled patientpopulation.
ThePractice Nurse will be responsible for a number of clinical areas such ashealth promotion, chronic disease management, and mentoring other members ofstaff. Actively supporting the practice management team in the reviewing anddeliveryof clinical policy and procedure.
AlbertRoad Surgery will ensure that the post holder has access to appropriateclinical supervision and an appropriate named individual in the organisation toprovide general advice and support on a day-to-day basis.
Person Specification
Qualifications
* Registered General Nurse Full NMC registration experience in chronic disease management.
* Mentor or teaching qualification Post graduate diploma or degree (Chronic Disease Management) Family Planning Qualification prescribing
Experience
* Experience of working in a GP Practice
* Primary Care Experience of chronic disease management
* Independent/ supplementary prescriber
* Interest in women health/ contraception/HRT with accredited qualifications
