You will be an enthusiastic and experienced nurse, with Community experience a warm personality and great people skills. You will need leadership experience and be able to demonstrate innovative practice.

Showing effective management skills, you will take responsibility for the day to day running of the community nursing service.

Main duties of the job

Day to day management of Community Nursing service

* Supporting staff to develop and enhance their capabilities
* Supporting colleagues in the delivery of compassionate, safe and structured care
* Work innovatively to ensure Community Service is fit for challenges and future demands
* Plan and provide treatment in collaboration with others as appropriate

* Community Nursing experience at Band 6.
* Have been involved with change management or innovative idea
* Sound communication skills in order to support challenging conversations.

Person specification

Education/Qualifications/Training

* Registered Nurse – Adult
* Specialist Practitioner qualification (DN Degree, diploma or Certificate in District Nursing) or equivalent clinical experience and leadership qualification
* Non-Medical Prescriber /Nurse Prescriber Qualification

Continuous Professional Development

* Active on the Mentor register
* Evidence of Leadership & Management development
* Teaching qualification/or willingness to undertake
* Recognised coaching qualification or willing to undertake

Additional Requirements

* Able to work flexibly across service areas to enhance quality of service provision
* Valid full UK licence with confidence to drive during unsocial hours and in unusual environments with use of car for work
* A degree of agility and the ability to manoeuvre light to moderate load
* Able to work bank Holidays, weekends and unsocial hours to meet service need
* Able to work in challenging environments
* Understanding of QMIS and Quality Improvement initiatives

Previous Experience

* Evidence of clinical leadership within a community nursing environment including coaching Teaching/assessing in practice, Recruitment and selection, Risk and Incident Reporting (Root Cause Analysis) and supporting teams in learning from findings.
* Community Nursing experience at Band 6 for significant time
* Able to conduct courageous conversations and support staff with performance related issues
* Able to support and manage own workload and support others

Knowledge, Skills & Abilities

* In depth understanding of case co-ordination and the role of Community Nursing
* Able to motivate self and teams to manage change
* Works collaboratively with their counterparts from teams in other provider organisations to develop a seamless service
* Effectively leads, manages and motivates a team by being highly visible, approachable and communicative at all levels; utilising effective written, oral and presentation skills.
* staff to undertake clinical duties implementing evidence based practice
* Ability to influence and problem solve
* Applies policies and procedures effectively to manage risk
* Manage performance through the application of HR processes
* Demonstrates flexibility to deal with the needs of unscheduled services and able to prioritise workloads and work under pressure

* Able to utilise IT and various media in order to support data collection and produce databases, spread-sheets and reports.
