Overview
Position: Frailty Pharmacist – St Helens South Primary Care Network. Location: Across St Helens South PCN, based at the Hub. Salary: Competitive, dependent on experience. Contract: Permanent.
Making a real difference to the lives of frail and elderly patients across our community. We are looking for an experienced and motivated Frailty Pharmacist to join our multidisciplinary team, delivering high-quality, patient-centred care for our population of over 85,000 people across 10 GP practices in St Helens South.
Responsibilities
* Conduct Structured Medication Reviews for frail or elderly housebound patients
* Tackle inappropriate polypharmacy and lead deprescribing initiatives
* Support end-of-life care planning and safe medicines management
* Work closely with GPs, frailty matrons, geriatricians, nurses, care coordinators, and social prescribers
* Provide medicines optimisation training and lead quality improvement projects
About You
* Completion of PCPEP Primary Care Pathway
* Minimum 2 years post-registration experience
* Skilled in medicines optimisation and polypharmacy management
* Experience in care homes or community settings
* Experience working with frail or elderly patients
* Confident communicator who thrives in a multidisciplinary team
* IT-proficient, ideally with EMIS
* Car driver with business insurance (preferred for community-based visits)
* Enhanced DBS Check required
* Right to work in the UK
