Hospital Discharge Administrator Hospital Discharge Administrator Location: Manchester-sited within one of the three hospital sites in Manchester (North Manchester General, Manchester Royal Infirmary, Wythenshawe Hospital) Hours: 35 per weekContract: Immediate start up to March 2026Salary: 24, 412 per annumBasic DBS check requiredWe’re looking for a compassionate and organised Project Administrator to support carers during hospital discharge. You’ll be the vital link between hospital teams and community services, helping carers access timely support and preventing carer breakdown. This role is key to ensuring smoother, safer transitions from hospital to home.What You’ll Do Build relationships with hospital staff to promote carer recognition and involvement in discharge planning Support a caseload of carers through the discharge process Conduct Carers Assessments and refer individuals to appropriate services Deliver training and awareness sessions to hospital teams Collaborate with the Manchester Carers support team and contribute to monitoring and reporting What We’re Looking ForWe’re seeking someone who is: Experienced in supporting vulnerable individuals and understanding carers’ needs Knowledgeable about safeguarding, statutory services, and community care structures Proficient in Microsoft 365 and client data systems Highly organised, with excellent communication and interpersonal skills Committed to equity, diversity, and inclusion Desirable Extras Degree-level education or equivalent experience Experience delivering training and conducting statutory Carers Assessments Familiarity with Greater Manchester’s VCSE sector and carer support landscape What you need to do nowIf you're interested in this role, please forward an up-to-date copy of your CV.
