Job Overview
Join our multi‑disciplinary care team as a Senior Community/District Nurse. This role requires a UK driving licence and access to a car. You will be responsible for managing a caseload, leading community staff nurses, and delivering high‑quality care within the 7 am‑11 pm service hours (including evenings, weekends and bank holidays). Flexibility and shift work within these hours are expected.
Key Responsibilities
* Manage a caseload of community patients, including assessment, care planning, implementation, and evaluation.
* Supervise and deploy staff, providing leadership, support and mentoring to junior team members.
* Deliver highly skilled nursing care to residents across West Essex, promoting health and independence.
* Collaborate with General Practices, Social Services and community partners to provide integrated care.
* Implement and monitor the Out‑of‑Hospital care model, ensuring service quality and adherence to clinical and management strategy.
* Act as an autonomous practitioner within the Community Nursing Team, supporting patients in their own homes.
Required Skills and Experience
* Band 6 clinical experience in community or district nursing.
* Strong leadership, communication and organisational skills.
* Ability to manage a caseload and lead a multidisciplinary team.
* Willingness to work flexible shift patterns and to use a car for patient visits.
* Evidence of effective teamwork with Primary Care Networks and Neighbourhood Teams.

Additional Information
All employees must hold a DBS check; the cost will be deducted from the first salary if you are newly appointed.
