Role Overview: Field Care Assessor As a Field Care Assessor with Home Support Matters, you'll play a vital role in supporting our care and support workers to deliver person-centred, safe, and effective care across our community. You'll act as the crucial link between customers, their families, our care teams, and external professionals, ensuring a seamless and high-quality service.
Hours: 37.5 hours per week, Monday to Friday, 9am to 5pm

Participation in an on-call rota (including every other weekend) with enhanced pay of £15.97/hour

Key Responsibilities: Conduct detailed care assessments and develop personalised support plans

Ensure accurate medication management and comprehensive care documentation

Perform regular audits to maintain high standards and compliance
