What's involved with this role: Temporary Family Support Practitioner – Social Care Reference no: AFC RQ1263532 Pay rate: £12.83 per hour PAYE Hours per week: 37 Monday – Friday, normal working hours This opening assignment is for 2-3 months City: LONDONDERRY Enhanced DBS required for this role International Police Check A Family Support Practitioner is required to work closely with children and their families, learning about their strengths and challenges and putting together an assessment of their needs. You will be a vital member of the team, skilled in providing a high level of professional childcare and social work practice using a wide range of short term (maximum 12 weeks) therapeutic interventions that are outcome focused, evidence based and maximise opportunity for change. WEISS aims to support families when difficulties arise before they need involvement with statutory services and seek to address issues of concern, achieve sustainable change, promote capability and capacity within families to problem solve difficulties and coordinate person centred, evidence based, early intervention for families with children 0-18 years old within Tier 2 of the Hardiker Model. WEISS is an outreach service working in families’ homes covering all areas of Derry/Londonderry and Strabane. It can include some early evening work to suit working parents. Key responsibilities: Work with families experiencing complex challenges and be creative in your approaches in engaging families who may be avoidant and resistant to your efforts. Provide reports to referrers which critically analyse progress and outcomes achieved and which will inform decision making on children’s and their families circumstances going forward. Provide a high-quality service to children, young people and their families, based on an assessment of need and is outcomes driven. Deliver complex and specialist interventions with children, young people or families on a 1:1 basis, group or community setting. Provide lead case/care planning management, including undertaking assessing, reviewing and managing risks Attend and participate at relevant multi agency meetings, providing a lead role ensuring reflective and analytical practice review. Take responsibility for day to day resource planning to ensure high quality services are delivered. Establish and develop effective relationships and channels of communication with children, young people, families, colleagues, community organisations, partner agencies and other external agencies. Retain accurate and contemporaneous records, using various IT systems to capture information and to produce written reports and present these as necessary. Experience: Experience of managing cases, in accordance with agreed procedures and completing assessments, planning and reviewing work to ensure positive outcomes for young people and families. Experience of providing services to families and children aged 0-18 years old. Minimum of 12 months recent experience in a similar role or context and ideally within a public sector organisation. Experience working within a multi-agency social care, early years or health environment and applying safeguarding procedures. Knowledge: A knowledge and commitment to safeguarding and promoting the welfare of children and young people. Working knowledge of an outcome-based approach to planning, delivering and reviewing services. Knowledge and practicable application of social care and other related legislation that impacts on the job purpose. Skills & Abilities: Skilled in overcoming the challenges in maintaining and developing effective working relationships with external agencies to ensure the best outcomes for children, young people and their families. Good recording skills, keeping records up to date and in a timely manner. Excellent communication skills to ensure best possible service delivery. Strong IT skills including Microsoft packages or equivalent (e.g. G Suite). Ability to work in a team and in partnership with various stakeholders and professional advisors and maintain good client relations. Able to work under pressure and have a problem-solving attitude with an ability to bring a range of interventions together to achieve positive outcomes. Excellent organisational skills and ability to prioritise tasks effectively, and manage time effectively. Ability to keep the child and or young people as the focus and ensure their feelings and wishes are represented in meetings and record keeping. Ability to undertake home /site visits ensuring compliance with lone working policies and procedures.
