Join Our Team as a Social Care Assessor – Liverpool City Council
Based in Liverpool | 35 hours per week | Initial 3-month contract with potential extension

We are currently looking for a Social Care Assessor to join one of our dedicated Locality teams. This is your chance to contribute to the delivery of high-quality adult social care services that support independence, dignity, and improved outcomes for adults across our city.
What You'll Be Doing
As a Social Care Assessor, you’ll play a key role in supporting adults with care and support needs by:
1. Conducting strengths-based assessments and co-designing outcome-focused support plans.
2. Championing the Prevent, Reduce, Delay principles of the Care Act to promote independence and wellbeing.
3. Managing a varied caseload while ensuring timely reviews and monitoring of support plans.
4. Working in partnership with individuals, carers, advocates, and partner agencies to deliver inclusive, person-centred care.
5. Upholding the council’s values and professional standards in all that you do.
What We’re Looking For
We’re seeking a compassionate, proactive individual who is:
1. Experienced in adult social care or has strong awareness of community care across statutory, voluntary, or private sectors.
2. Skilled in assessment, support planning, and multi-agency working.
3. Organised and resilient, with the ability to manage fluctuating priorities while delivering high-quality work.
4. IT confident and comfortable using systems such as Liquid Logic and agile working tools.
Qualifications & Experience:
1. Literacy, numeracy, and basic IT skills (essential).
2. NVQ Level 3 or 4 in Health & Social Care or Promoting Independence (desirable).
3. Prior experience in social care settings and working with diverse communities is a plus.
