Health Care Assistant - District Nursing

As a Community Health Care Assistant working in the Out of Hours District Nursing Service you will be integral to the care of the patients.
The Trust is focused on supporting the Community and helping patients remain independent in their own home. You will actively assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting, including supporting end of life patients.

The Evening and Overnight district nursing team supports patients from 19:00-08:00 across a 7 day week, 365 days a year.
Due to the nature of this post you will require a full clean driving license with access of a vehicle.
The core hours of this post is 20 hours x 4 evenings per week 19:00-00:00 across 7 days covering weekends and bank holidays.
Main duties of the job
You will work alongside a member of the team, working in pairs for safety with full support of your Team Leader and other members of the team. You will provide treatment to variety of patients with health and social care needs. This can vary from simple and complex patient care to looking after people at the end of their lives who wish to remain at home. You will work closely with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patient care is appropriately coordinated.

Person Specification
Qualifications
* NVQ Level 3 in Healthcare or a Level 3 Senior Healthcare Support Worker Apprenticeship - or equivalent experience
* Level 1 Literacy and Numeracy (GCSE English and Maths Grade D-G)
* Care Certificate
Knowledge, Training & Experience
* Experience of working as a Care Support Worker or equivalent in a Health or Social care setting
* Experience of working within a team
* Demonstrates an interest or previous experience in supporting other health care staff and learners
* Commitment to undertaking continuous professional development (CPD)
Skills & Abilities
* Good communication (written and verbal skills)
* Basic IT skills
* Full UK driving license with willingness to use own car for work purposes
* Ability to travel to different geographical locations across the Trust footprint in line with service needs
