An exciting opportunity has arisen due to the expansion of the Tissue Viability service. You will manage the evidence-based Tissue Viability Service at the Countess of Chester Hospital NHS Foundation Trust as part of a multidisciplinary approach to patient care.
The post holder will develop the service across the organisation, working within clinical governance guidelines and protocols. Responsibilities include improving, maintaining, and monitoring Tissue Viability through audits, surveillance, education, and strategic planning.
You will work autonomously, fostering collaborative relationships across professional boundaries, acting as an expert resource in Tissue Viability, and providing support, education, and leadership.
The role involves delivering high-quality, expert nursing care, assessing referrals, managing patient care plans, and prioritising cases based on clinical need. You will be responsible and accountable for all aspects of patient care in this specialist area.
The Trust serves West Cheshire and Welsh patients, collaborating within the Cheshire and Merseyside Health and Care Partnership. It operates from three locations:
* The Countess of Chester Hospital: 438 beds
* Ellesmere Port Hospital: 56 beds
* Tarporley War Memorial Hospital: community services base
