Responsibilities
* Running chronic disease management clinics (e.g. Asthma, COPD, Diabetes, Hypertension)
* Providing immunisations and vaccinations for children and adults, including travel and seasonal vaccines
* Carrying out cervical cytology screening
* Undertaking wound care, dressings, suture removal and minor injuries work
* Conducting health checks, ECGs, spirometry and other diagnostic tests
* Supporting patients with health promotion, lifestyle advice and self-management
* Working closely with GPs, nursing colleagues, HCAs and the wider practice team
Qualifications and Skills
* A Registered General Nurse (RGN) with at least 2 years post-registration experience
* Skilled in immunisations, wound care, cervical cytology, and patient assessment
* A good communicator, compassionate, reliable, and a real team player
* Motivated to learn and develop, with a passion for improving patient care
* Experience in primary care or chronic disease management is desirable, but not essential; training and development opportunities will be provided for the right candidate.
