Weare looking to recruit an enthusiastic and proactive Medicines Administrator. Thepost is for 25 hours per week, spread across five days (Monday to Friday), toensure daily coverage and continuity.
Thisis a fantastic opportunity to shape and develop what is a new role added in 2025that will be central to our medicines management processes. You will workclosely with our clinical team, patients, and community pharmacies to ensuresafe, efficient, and timely prescribing.
Theposition will help the practice streamline the process of medication reviewsand the associated prescriptions, thus improving the experience for bothpatients and GPs and hopefully, reducing the need to contact reception forassociated matters, allowing other patients to get through.
Weare looking for a driven individual with a keen attention to detail with a cando attitude, capable of thinking proactively whilst operating within the scopeof procedures.
Main duties of the job
Maintainclear, compassionate, and professional communication with patients to ensureunderstanding of medication regimens, address queries, and promote adherence
Liaiseeffectively with GPs, nurses, pharmacists, and administrative staff to supportsafe prescribing, accurate medication administration, and continuity of care
Promptlyescalate concerns or discrepancies related to medicines to the appropriateclinical staff to uphold patient safety and best practice
Manageand streamline the repeat prescription process and co-ordinate requestsfor acute items
Arrangerelevant monitoring to support clinical reviews
Runsearches to identify patients due for medication reviews and arrange anyrelevant monitoring such as blood tests, blood pressure checks etc
Runsearches for patients on high-risk drugs requiring more frequentmonitoring and identify any issues i.e. non-compliance and escalate to theclinical team
Monitoringpatient test diary on our clinical system EMIS to facilitate continuity ofcare
Performdata quality checks to ensure timely and appropriate reviews
Supportongoing rollout of the Electronic Prescription Services (EPS)
Supportpatients with signing up to the NHS Wales App to improve usage
Workwithin scope of role as outlined within medicines management standardoperating procedures

Person Specification
Experience
* Demonstratable experience working with others as part of a team
* Able to communicate effectively with colleagues and patients
* Experience of working in Primary Care
* Knowledge of prescription process
Qualifications
* Excellent organisational and communication skills
* Good IT skills are essential. Strong IT skills would be an advantage, but training and support will be provided for the right candidate
* Ability to work effectively within a team
* A proactive, can-do attitude with a willingness to develop the role
* Experience in primary care or community pharmacy
* Familiarity with prescription processes or medicines management
* Experience and confidence using clinical IT systems
* Welsh speaker
