Job Description:
TRC has been retained exclusively by one of the leading providers of specialist complex care in Cornwall.
Their services include person-centred nurse-led care for adults and children with complex physical disabilities and life-limiting conditions, ensuring each individual reaches their full potential.
This role involves overseeing the clinical aspects of care packages within the area.
70% field-based / 30% home-based
Client needs may include:
* Long-term conditions (e.g., Cerebral Palsy, MND, Ehlers-Danlos Syndrome, MS)
* Ventilation
* Paediatric complex care
* End of life care
* Medication management
* Diet and nutrition management
Key Responsibilities:
* Implementation of new care packages
* Re-assessment of clinical tasks and requirements
* Leading monthly team meetings
* Care planning and risk assessments (initial and ongoing)
* Conducting new client meetings as required
* Medication and clinical audits
* Providing clinical supervision for Registered Nurses on care packages
* Competency sign-off and management
* Clinical liaison with the MDT as needed
* Providing standard and emergency support to regional Care functions
* Providing clinical oversight in line with best practices, policies, standards, and legislative requirements under CQC
* Supporting the Operations Manager and Clinical Management within financial plans
* Supporting care teams to meet personal care needs, including bathing, washing, and supporting with mealtimes
* Training care staff in the use of medical equipment
* Working within the company's structure and ethos, aligned with mission, vision, and values
* Developing effective relationships with care and management teams
* Maintaining current clinical skills and undertaking ongoing training
* Supporting staff development through spot checks and feedback
* Daily travel within the role
Requirements:
* Registered Nurse / Midwife with current NMC registration or Registered Allied Health Professional with current HCPC registration
* Experience interviewing healthcare professionals (registered and unregistered)
* Healthcare experience required
