Job Summary:

Main duties of the job

Key Duties/Responsibilities


* To undertake clinical assessment and provide treatment for patients within the defined group
* To complete frailty reviews as required
* Link with clinical pharmacists to undertake structured medication reviews
* Link with other services to reduce admissions to secondary care
* To support frail and elderly patients
* To receive referred patients from other clinicians and triage services and provide necessary treatment and advice
* To undertake comprehensive reviews and develop individual care plans for all residents
* To plan and implement interventions and treatments for patients with acute/chronic conditions
* To be able to prioritise caseload and intervene appropriately in complex, emergency or urgent situations
* Initiate effective emergency care, when required
* Build strong relationships across the practice
* To prioritise own workload
* To respond in a timely manner to the changing needs of patients and the service, in terms of the scope and nature of clinical practice
* To work remotely, as required, undertaking ward rounds via video or telephone consultation
* To work within the LMP visiting service multi-disciplinary team
* Chair multidisciplinary meetings

* Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
* Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
* Maintain professional registration
* Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards
* Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation
* Pro-actively promote the role of the Frailty Matron within the Practice, and externally to key stakeholders and agencies
* Work with the team to investigate adverse clinical incidents and complaints.

Qualifications

Essential

* Adult nursing degree, or equivalent
* Relevant clinical professional qualification
* Registered RGN
* Continual Professional Development and CPD evidence / portfolio
* Experience of working autonomously and as part of a team
* Experience of Implementing protocols and clinical guidelines
* Experience of producing and analysing audits
* Minimum 2 years experience since qualification
* Understanding of the principles of case management
* Awareness of the current national and local agenda in health and social care for frailty
* Strong communication skills, both written and verbal
* Ability to work without direct supervision and determine own workload priorities
* Ability to show empathy along with excellent observational skills.
* Ability to plan and prioritise workload
