The core hours are Monday to Friday 9-5, with some flexibility considered. This is an autonomous post within the North Manchester Specialist Community Mental Health Teams, based in the Central Park Hub, designed to deliver safe, effective and high-quality services to service users in a vibrant MDT environment.
We are seeking a dynamic, compassionate Registered Mental Health Nurse with strong clinical skills to support the SCMHTs and contribute to assessment and treatment for patients accessing the service.
As a qualified nurse you will demonstrate and live the NMC values as identified in the Code of Practice, and work to uphold the Trust values of inspiring hope, working together, being caring and compassionate, valuing and respecting others, and being open and honest.
Your role will include using research-based interventions to promote independence and recovery while maintaining high quality clinical documentation, and respecting individuality, cultures and diversity in care.
Responsibilities
* To promote Positive Mental and Physical Health to individuals under the care of the service in all situations.
* To ensure and support individuals to collaborate in the development of their recovery plans and structured care plans and treatment.
* To assess, plan and review in line with requirements of the Care Act (2014) and s117 of the Mental Health Act (1983).
* To provide clinical supervision to members of the multi-disciplinary team based on your clinical and professional knowledge.
* To supervise junior members of staff in their clinical risk assessment and formulation of crisis and risk planning.
* To promote recovery focused care and interventions for individuals.
* To ensure that all interactions and documentation is person-centred following the NHS guidance No decision about me, without me.
* To co-produce individualised recovery plans with the individuals, carer or others identified by the Service User.
* To promote and maintain a learning culture within the team which supports the professional and effectiveness of clinical care.
* To contribute to the delivery of multidisciplinary training including the induction of new staff, students and trainees.
* To demonstrate a high level of active communication skills with individuals and in a variety of forums and situations.
* To utilise highly developed interpersonal skills to facilitate effective communication and overcome barriers arising from sensory or cognitive impairment, hostility or highly emotive situations.
* To provide specialist Mental Health Nursing interventions in the assessment, support and recovery of individuals with severe mental health difficulties, including Mental State Assessments, Social Care Assessments, physical health monitoring and side effect monitoring.
* To ensure all individuals have clear and accurate information regarding their care or the care of loved ones.
* Where required, to act up in the absence of the senior practitioner.
* To seek line management and clinical supervision as required.
* To involve individuals and their carers in assessment, formulation and review of care plans where practicable.
* To maintain high standards of clinical recording, ensuring information is accurate, appropriate and up to date.
* To comply with GM Mental Health NHS Foundation Trust policies on confidentiality, information governance and professional practice.
* To maintain personal well-being by using resources available via the Trust or professionally.
* To comply with health and safety legislation and the welfare of yourself and the wider Trust.
* To identify unmet needs to support service improvement or planning to enhance individuals' experiences of health services.
* To comply with the NMC code of professional conduct and local/national professional standards, policies and procedures.
* To adhere to safeguarding principles for children and vulnerable adults and undertake safeguarding enquiries as appropriate, ensuring person-centred, proportionate actions and risk mitigation.
* To ensure colleagues and junior staff are competent and compliant with safeguarding legislation and guidance.
* To follow Infection Prevention Control policies and maintain high standards of IPCC.
* To comply with service security requirements, lone working procedures, and risk management procedures.
* To manage risk in accordance with Trust policies, including investigation processes via the Inphase system.
* To uphold the Trust values and work in an environment free from harassment, discrimination, abuse and violence.
* To promote the Trust’s Anti-Racism agenda and maintain candour in practice.
* To develop and maintain good working relationships with partners and community services including GPs, commissioners and third sector agencies.
* To promote awareness of mental health and the CMHT's role within local services and the wider community.
* To advocate on behalf of individuals to ensure involvement in their care and treatment.
* To exercise effective time management, punctuality, consistency and reliability.
* To demonstrate empathy and compassion to individuals or families who may wish to make a complaint about their care.
* To pursue ongoing learning and professional development and act as a Practice Educator for students on clinical placements.
* To maintain professional registration and inform the Trust of any changes or restrictions.
* To only access clinical information as required for your duties.
* To maintain confidiential information securely and disclose only as justified with permissions and need-to-know basis.
* To undertake any other reasonable duty as requested by a Trust manager.
* To understand and comply with all Trust policies, procedures, protocols and guidelines, and the Trust’s strategic goals.
