Job summary
An exciting opportunity has arisen for an additional part time care coordinator tojoin East Lindsey Primary Care Network as part of the proactive, falls andfrailty Occupational Therapy (OT) team in our south practices. The OT team support patients withinEast Lindsey Primary Care Network to maintain independence with activities ofdaily living, reduce the risk of falls, reverse or prevent frailty progressionand promote wellbeing. Our aim is to be as proactive as possible to reduce thechance of people reaching crisis situations. The Occupational Therapy CareCoordinator will be invaluable in helping us to deliver these aims.
The role is varied and will include supporting the Occupational Therapists tocomplete functional assessments and follow-up visits in GP surgeries, patientsown homes, care homes and via telephone. You will be involved in supporting todeliver intervention plans which may include the demonstration of exercisesprescribed by the Occupational Therapist. You will prescribe basic equipment toaid activities of daily living and support with follow-up assessments once theequipment is in situ. You will also support in the coordination and completionof onward referrals for patients to ensure they receive appropriate,well-coordinated care.

SupportingOccupational Therapists to carry out functional assessments and follow-upvisits within the patients own home, within the service remit
Deliveryand fitting (where required) of assistive equipment as directed by theOccupational Therapist and safe moving and handling of the same
Tomaintain a personal duty of care.
Tobe responsible for the correct and safe use of the physical environmentincluding equipment and to be responsible for the allocation of these resources
Activelyparticipates in, in-service training opportunities
Effectiveand accurate communication with patients, families and the multi-disciplinaryteam regarding outcomes of assessments and visits within scope of the job role
Makingonward referrals to health and social care providers as required usingappropriate IT systems
Inductionand training will be provided along with team support, supervision and trainingopportunities.
