Social Worker Community Learning Disability Team
An exciting opportunity has arisen for a social worker to join the Learning Disability Service in Salford. The Learning Disability Service has been an integrated team for over 20 years and the successful candidate will work alongside health colleagues in a single service.
We strive to support people to live well and be safe at home, promoting their wellbeing and helping them achieve personal outcomes through strength‑based conversations. We work closely with partner organisations to ensure people are well connected with their local communities.
Responsibilities
* Complete strength‑based assessments and promote well‑being using resources from social care, health and other voluntary or statutory agencies.
* Undertake initial assessments and proactively work with complex and legal issues, crisis planning, providing information, advice, signposting and transferring to other social work teams or services as required.
* Carry out safeguarding functions, including initial information gathering, risk assessment, safety planning and determining if Safeguarding Adult Enquiries under section 42 of the Care Act are required.
* Manage competing demands and prioritise work effectively, staying calm under pressure.
Qualifications
* Degree in social work.
* Registered with Social Work England.
* Evidence of continuing professional development.
* Strong understanding of the application of the Care Act, Mental Capacity Act and Safeguarding Adults.
