The Community Urgent Response Team (previously known as Single Point of Access) service is continuing to develop and change in order to meet the needs of the local population. The service supports clinical triage, ensuring service users are signposted to most appropriate service to meet their needs.
The hours are varied and include a mixture of shift patterns to support 24-hour working seven days a week.
The post would suit candidates who have previously had patient contact and who have sound administration skills and are confident utilising electronic patient records. Experience with telephone triage would be desirable but not essential.
The ideal candidate would enjoy working in a team and be motivated and enthusiastic.
The Main Responsibilities Are
* To provide an effective single point of referral and call handling service for the community services provided by NLaG
* To make supported decisions on the most appropriate care pathway and direct service users/professionals into the appropriate service area ensuring equality of access across a range of services
* To carry out back-office functions as required relating to the timely processing of referrals and to ensure patient records are accurately and quickly updated
* To provide coordination of visits and monitoring of the referrals
* To work closely with and support the Community Urgent Response nurses and Wider Community teams
* To take referrals for community services.
