Hourly Rate: £33.00 per hour

Main Responsibilities
* Deliver high-quality social work services to adults and families, focusing particularly on complex cases.
* Conduct comprehensive needs assessments and create tailored care and support packages.
* Balance risk and autonomy to promote safe and independent living for service users.
* Provide expert advice and support to colleagues, students, and other professionals.
* Promote independence and informed decision-making through collaboration with individuals, families, and communities.
* Design and commission cost-effective care solutions.
* Engage in preventative interventions using tools such as reablement and telehealth.
* Maintain accurate and timely records on both electronic and paper systems.
* Present cases to the Resource Allocation Panel and contribute to legal proceedings where necessary.
* Build positive working relationships with service users, carers, and partner agencies to gather information and resolve conflict.
* Stay up to date with current legislation, policy, and best practices within adult social care.
Essential Skills and Knowledge
* In-depth understanding of the Making Safeguarding Personal approach.
* Solid working knowledge of the Mental Capacity Act 2005, Human Rights Act 1998, and Care Act 2014.
* Strong professional ethics and values focused on empowering individuals and achieving positive outcomes.
* Ability to work preventatively and collaboratively with families, carers, and the wider community.
* Experience in creating efficient, value-for-money care plans and liaising effectively with multidisciplinary teams.
