TheSouth Islington Primary Care Network (PCN) is a group of 7 independent GPpractices in the South of Islington, covering a population of around 75,000residents. The PCN is brought together by NHS Englands PCN DES contract todeliver against its specifications and other tasks. The PCN works with multiplestakeholders as part of the South Locality, a multi-agency group focused onidentifying population health needs and identifying streams of work to supportthese. This is part of Islingtons Fairer Together borough partnership.

Thepopulation within the PCN geography is diverse, often with significant andcomplex health and wellbeing needs. There is an exciting opportunity forenthusiastic and motivated individuals to support the co-ordination of thiscare and working closely with people to support them to achieve improved healthoutcomes and a better quality of life.

Asthis is a new role and the guidance around PCNs is constantly changing, thereis a need for the post holder to be flexible and adaptable in this evolvingenvironment. As a consequence, the post will be subject to ongoing review andinevitable iteration (the post holder may be requiredto work at other sites as dictated by the needs of the service),and will help inform how roles like these can be effectively used to supportpatient care.


Main duties of the job

To apply, you should be afully HCPC and CSP registered physiotherapist with at least 5 yearspost-graduate experience, including 3 years of relevant MSK clinical experiencein line with HEE's Roadmap to Practice.

You should be a confidentMSK practitioner and communicator who thrives from engaging with a variety ofcolleagues internally and within the GP practice multidisciplinary teams youare integrated into.

Thesuccessful candidate will play a key role in proactively working with people,who would need support around MSK-related conditions and to providecoordination and navigation of care and support across health and careservices.

Theywill work closely with GPs and practice teams, making sure that appropriatesupport is made available to people; supporting them to understand and managetheir condition and ensuring their changing needs are addressed. They willenable people to access the services and support they require to meet theirhealth and wellbeing needs, helping to improve peoples quality of life.

JobPurpose

Provideclinical expertise, acting as a First-Contact Physiotherapist and makingdecisions about the best course of action for patients' care (including inrelation to undifferentiated conditions. This will involve seeing patients,without prior contact with their GP, in order to establish a rapid and accuratediagnosis and management plan.

Toact as a clinical role model providing leadership and support to a team ofclinical staff delivering care at GP surgeries across the South Islington PCN

Reducelocal health inequalities

Enable patient access to MSKPhysiotherapy at the start of the pathway, ensuring timely access to diagnosis,early management and onward referral if necessary

Support patients to stay well and makeinformed decisions and choices through shared decision making when their healthchanges

Empower patients to build knowledge,skills and confidence to live well with their health conditions

Job Role

Responsibilities will include, but are not limitedto:

Work within primary care, seeing patientsdirectly from reception booking ad triage.

Assess, diagnose, triage, and manage patients,taking responsibility for the management of a complex caseload.

Where required refer patients to other healthprofessionals within the PCN

Work as part of a multi-disciplinary team in apatient facing role, using their expert knowledge of movement and functionissues, to create stronger links for wider services through clinicalleadership, teaching and evaluation

Develop integrated and tailored care programmesin partnership with patients, providing a range of first line treatment optionsincluding self-management, referral to rehabilitation focussed services andsocial prescribing

Make use of their full scope of practice,developing skills relating to independent prescribing, injection therapy andinvestigation to make professional judgements and decisions in unpredictablesituations, including when provided with incomplete or contradictoryinformation.

Manage complex interactions, including workingwith patients with psychosocial and mental health needs, referring onwards asrequired and including social prescribing when appropriate

Communicate effectively with patients, and theircarers where applicable, complex and sensitive information regarding diagnoses,pathology, prognosis and treatment choices supporting personalised care

Implement all aspects of effective clinicalgovernance for own practice, including undertaking regular audit andevaluation, supervision and training

Develop integrated and tailored care programmesin partnership with patients through:

-Effective shared decision-making

-Assessing levels of Patient Activation to supporttheir own level of knowledge

-Designing and implementing plans that facilitatebehavioural change

Request and progress investigations such asx-rays and blood tests and referrals to facilitate the diagnosis and choice oftreatment regime including, considering the limitations of theseinvestigations, interpret and act on results and feedback to aid patientsdiagnoses and management plans

Be accountable for decisions and actions viaHealth and Care Professions Council -HCPC registration, supported by aprofessional culture of peer networking/review and engagement in evidence-basedpractice.

Provide feedback and support to other PCNclinical team members on appropriate referral criteria.

Person Specification


Experience

* Knowledge of managing patients with a variety of conditions and complex needs
* Demonstrates expert clinical reasoning skills in MSK physiotherapy
* Ability to implement evidence-based practice principles
* Good self-management and ability to prioritise workload and delegate tasks as appropriate
* Evidence of appropriate post-graduate training in a broad range of appropriate specialist areas
* Evidence of an active part in learning activities e.g. peer review
* Evidence of providing professional advice /support
* Evidence of collaborative working within a multi-professional team
* Experience of working in Primary Care and First Contact Practitioner roles
* Evidence of involvement in Service Improvement initiatives and outcomes
* Experience of initiating change
* Previous experience working in Primary Care
* Ability to cope with unpredictable situations
* Completion of some level 7 MSc learning. Qualified injection therapist and / or independent prescriber


Knowledge and skills

* Be flexible and adaptable to cope with unpredictable situations
* Awareness and understanding of the MSK Competency Framework for FCPs
* Ability to implement evidence-based practice principles
* Ability to manage a case load of patients independently with complex musculoskeletal dysfunction, acute and chronic pain
* Good self-management and ability to prioritise workload and delegate tasks as appropriate
* Ability to structure and deliver training and education programmes
* Computer literate for record keeping, communication, data collection and research purposes
* Aware of own limitations and when to ask for advice from the wider MDT
* Experience of working and have a good understanding of EMIS clinical platform


Qualifications

* BSc degree in Physiotherapy
* Registration with the HCPC
* Membership of the Chartered Society of Physiotherapy
* Minimum of 5 years post qualification experience
* Minimum of 3 years MSK experience
* MSc degree in Physiotherapy
* Completed or working towards a Supplementary Prescribing qualification or non-medical prescribing qualification training will be supported in post.
* Completed or currently working towards IRMER qualification
