Job Description
The purpose of this role is to be part of a small team of senior nurses responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace.
Key Responsibilities:
* Act as the primary contact for patients, families, professionals, and the community, managing all clinical enquiries.
* Provide clinical oversight and triage for referrals to ensure timely and appropriate services.
* Develop and enhance the nursing role within the Coordination Centre to deliver high-quality, responsive, and evidence-based services.
* Work as a Specialist Palliative Care CNS in various clinical settings monthly to maintain and develop skills.
* Conduct audits and case reviews to improve service quality.
* Contribute to strategic development and implementation of local and national hospice care strategies.
* Represent Rennie Grove Peace within the community.
Qualifications & Requirements:
* Demonstrated ability in clinical coordination and palliative care.
* Right to work in the UK; sponsorship not available.
