Senior Rehabilitation Assistant – Neuro Early Supported Discharge Team
We are looking for an enthusiastic and motivated Senior Rehabilitation Assistant to join our multidisciplinary Neuro ESD Team which provides specialist intensive rehab to clients with acute stroke and other neurological conditions following discharge from hospital or as community admission avoidance. The role involves working alongside physiotherapists, occupational therapists, speech & language therapists, a clinical neuropsychologist, rehabilitation assistants and admin staff across the Southampton and West Hampshire areas. Care is delivered in the client’s own home or alternative community setting.
Working Patterns
Monday–Friday with occasional weekend work. Flexible working options can be discussed as part of the interview process.
Responsibilities
* Provide senior rehabilitation assistant intervention to a specified caseload of patients with stroke and neurological diagnoses within a multidisciplinary team, delivering an intensive neuro‑rehab programme based on individualised patient goals.
* Assist clients with rehab programmes, including prescribed exercise programmes (individual and small groups), activities of daily living, mobility and transfer practice.
* Educate and teach clients and carers to enable self‑management and empowerment for optimal outcomes.
* Observe and report client progress against planned goals, alerting to achievements or deterioration during routine hand‑over meetings or urgently when required.
* Perform safe moving and handling practices daily, in accordance with planned programmes of care and Trust policies.
* Fit and operate basic equipment (e.g., walking frame, toilet seat raise) once competent, and train clients, carers and other staff in safe use.
* When in the community setting, work as a lone practitioner with telephone support and seek advice from experienced colleagues for matters beyond your scope of knowledge or skill.
Qualifications
* Foundation Degree/Diploma or NVQ Level 3 with experience equivalent to a foundation degree level.
