About the Role
A Cancer Pathway Navigator within the Colorectal Team will work with the Colorectal Triage and Colorectal Nurse Specialist, acting as a patient advocate and providing support and information throughout the entire cancer pathway to reduce patient anxiety.
Key Responsibilities
* Provide reassurance and information on appointments, interventions and tests, ensuring patients arrive for interventions fully prepared.
* Work closely with the MDT coordinator to support and coordinate the care of cancer patients through their complex care pathways.
* Ensure all patients move through the pathway without unnecessary delays to achieve a definitive diagnosis by day 28.
* Track patients on cancer surveillance as part of the introduction of Patient Stratified Follow Up (PSFU).
* Work autonomously, demonstrating strong communication skills and a commitment to patient compassion and quality of care.
* Competently collate and input data, demonstrating excellent IT skills across a broad range of systems.
* Handle complex situations with service users and clinicians, both in primary and secondary care, over the phone and in person.
Qualifications & Experience
Applicants should be competent in data collation and input, possess excellent IT skills, and have strong communication skills. Experience in patient care and ability to work autonomously are essential.
