The post is a full-time Band 7 Senior Children’s Nurse position within the Bexley and Greenwich Community Children’s Nursing Team and integrated Hospital at Home Team. The team provides a seven-day service from 08:00 to 20:30, with 24-hour on-call provision for children and young people who require end-of-life care at home. The CCN/HAH is part of an integrated network of services for children in Bexley and Greenwich, delivering high-quality, child-centred nursing care in children’s homes or community settings.

Responsibilities
* Provide senior leadership within the Community Children’s Nursing Team and integrated Hospital at Home Team, including deputising for the team manager when required.
* Clinically lead on service projects and developments, facilitate engagement of the team in change management and service innovations.
* Deliver seven-day community nursing services from 08:00 to 20:30, with access to 24-hour on-call end-of-life care at home.
* Work as part of a multidisciplinary team to provide high-quality, child-centred care in community or home settings.
* Demonstrate up-to-date academic development post-registration and extensive experience in community children’s nursing.
Qualifications
* Registered Children’s Nurse with extensive experience in Community Children’s Nursing.
* Evidence of up-to-date academic development post-registration.
* Ability to act as a role model and lead on service improvements and change management.
