Monday (Care Homes and Home Visits/ QOF Reviews) 8.30 to 16.30
Tuesday (Care Homes) 8.30 to 16.30
Thursday (Practice Clinic) 8.30 to 16.30
Friday (Practice Clinic/ Care Homes) 8.30 to 16.30
Must be able to drive and have your own transport.
Must have the right to work in the UK and be able to provide the documentation.

The main focus for the role will be to be an integrated part of our PCN Care homes and frailty team and our Proactive Care Team. You will work under the supervision of our Frailty Team Leader who is a very experienced Advanced Nurse Practitioner. Travel between 3 surgeries on a rota basis, Care Homes and Housebound patients. Dedicated time to Care Homes and Housebound patients. Experience of rota work. You will also be sole working as well as working as part of a team.
Working alongside other clinical staff in the provision of patient care and under the supervision of a registered nurse.

Job responsibilities
Undertake and record the following tasks.
* Urinalysis
* Measurement of blood pressure, temperature, and pulse
* Peak flow measurements
* New patient health checks
* NHS Health Checks over 40s
* Experience with ICE
* Diabetic Reviews Part 1
* Venepuncture
* Organisation of housebound annual bloods and entering health monitoring data via System one into patients records
* MMSE tests
* Performing ECG's
* Organisation of urgent blood tests for housebound patients at short notice
* Recognise changes in patients' observations which must be reported and acted on
* Assist clinicians in emergency management of patients
* Enable patients to access appropriate professionals in the practice team
* Act as a chaperone
* Provide relevant health promotion literature to the patient
* Maintain clinical equipment efficiently
* Instruct patients in appropriate techniques for medical equipment
* Interact with patients and colleagues in a professional manner
* Desired Covid Vaccinator Can be trained
* All surgeries work differently- System one experience preferred.

Experience
* Skills, knowledge and experience
* Knowledge of the Personalised care approach
* Understanding of the determinants of health to include social, economic and environmental factors
* Understanding of, and commitment to, equality, diversity and inclusion
* Knowledge of IT systems to include the ability to use word processing skills, emails and internet
* An understanding of the nature of general practice and Primary Care Networks
* Ability to build and maintain relationships whilst maintaining appropriate professional boundaries
* Excellent verbal communication skills with the ability to communicate effectively at all levels inc. customers, patients and carers, specialist services, GPs and colleagues.
* Listening skills displaying empathy.
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences
* Experience of using GP clinical systems, such as System One
* Good technical literacy of Microsoft Applications e.g. Word, Excel, and Access etc.
* Experience of working in primary care
* Experience of working in a GP practice
* Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (can include unpaid work)
* Knowledge of public health issues in the local area
