Job responsibilities Provide leadership and overall management expertise to the service, driving change and improvement by supporting and influencing the team and the corporate management team Ensure that the ICB meet their statutory requirements in relation to NHS Continuing Healthcare and other legislation in regard to Complex Care (e.g. Mental Health Act and DoLS). Determine and implement operational strategies and plans related to the overall CHC service Hold the strategic view on the commissioning of Continuing Healthcare and Complex Care and will be required to work closely with the ICB to ensure that the provision meets the changing requirements. The post holder will be required to achieve against a range of objectives and priorities related to the overall management of the patient pathway. Performance will be monitored via the ICB Quality & Performance Framework and against action plans developed in response to requests from ICB and from external regulatory bodies The role has the overview of the NHS continuing healthcare service across the local place and is responsible for promoting and delivering a consistent approach, manage the changes in process and improvement in service delivery, working in partnership with a range of external partners and providers to ensure the delivery of a high quality CHC service. Oversight and responsibility for commissioning and approval of new services for Continuing Health acre (CHC), Discharge to Assess (D2A) and Personal Health Budgets (PHB). Oversight and management of CHC appeals process and adherence to required timescales and manage contentious situations in relation to disputes between partner agencies or with individual patients and relatives. Act as a visible and accountable leader within the local health and social care economy to promote good practice in the Continuing Healthcare, Discharge to Assess and Personal Health Budget pathway across partner organisations including timely access and decision-making To be the expert lead on NHS Continuing Healthcare at the place, giving advice and input where requested. Work collaboratively across the wider ICB with Heads of CHC service at place and also with key stakeholders including, but not exclusively, Local Authority, NHS Trusts, third party providers, service user and carers groups To hold lead responsibility for ensuring that all processes implemented are compliant with legislative and national guidance and in accordance with the responsibilities of the ICB. Ensure compliance with best practice guidelines, risks are proactively managed and issues identified, investigated and resolved. Support the Chief Nursing Officer by contributing to the strategic planning of the CHC function within the ICB, as well as providing technical advice to improve capacity planning and delivery Support productive and positive whole systems working in order to ensure service resources are deployed effectively to deliver a responsive, high quality and an excellent patient experience Evaluate performance of CHC and Complex Care assessments to ensure quality, consistency and maintenance of best practice within changing requirements. Ensure training is established for all Continuing Healthcare staff to achieve on-going improvements in the system and keep abreast and disseminate policy changes. Represent the place, ICB at regional and national conferences and on external working groups as appropriate To create a vision for the CHC service that is aligned to the delivery of the ICB corporate objectives and which links with the overall strategy for services Ensuring the strategic development of a patient pathway which is safe, efficient, effective and economic, and which contributes fully to the ICB priorities and targets Overall responsibility for the delivery of agreed performance metrics as required by the Chief Nursing Officer and to support the delivery of key national targets Authorising of individual care packages for Continuing Healthcare eligible patients up to the delegated authorisation. Ensuring that oversight of the budget is maintained, data and trends are identified, solutions and actions are agreed to ensure that financial balance is achieved Authorisation for specialist health equipment for non CHC patients within delegated budget Responsible for the budget for Continuing Healthcare, Funded Nursing Care and Complex Care. This will include participation in budget setting, procurement and overall budget management. Provide in depth analysis, interpretation and production of complex and multiple reports including financial returns internally and external organisations (NHSEI) in relation to performance, quality and activity Accountable for ensuring there is a plan in place to improve staff engagement and satisfaction for line managed staff with a focus on improvement of staff survey results Provide both strategic thinking and technical advice to develop and implement innovative capacity plans and improved processes To encourage service user/carer feedback on the service provided and to improve/develop the service in response to this feedback Lead on the development of quality, performance and governance strategies and the development and implementation of improvement programmes, in accordance with national and local priorities Responsible for developing policy and procedures that are necessary to ensure the safe, robust and high-quality delivery of the NHS Continuing Healthcare service, including briefings, commissioning, business or resources Works with key partners to develop joint policies and procedures which will enhance the overall delivery of service-to-service users and enable closer working practices
