Offer description

Overview

NHS Fife has a fantastic opportunity for a pharmacist to join our rapidly developing, friendly primary care team in Kirkcaldy. This team supports GP practices to deliver polypharmacy reviews, medicines reconciliation, acute and repeat prescription requests, medication reviews and other work in line with the GMS contract. There is opportunity for full utilisation of prescribing on a daily basis within the team.

The role involves working with GP Practices as part of a multi-professional team to optimise safe, quality, evidence-based and cost-effective use of medicines. Pharmacists, and the wider pharmacy team, are embedded as part of the multi-disciplinary team in all practices, to help deliver high quality pharmaceutical care, ensuring patients get the best possible outcomes from their medicines, avoiding harm and waste.

You will be supported by Senior Practice Pharmacists and will work with an expanding and developing team of fellow Practice Pharmacists, Pharmacy Technicians and Medicines Management Support Workers.

Whilst the position on offer is full time (37 hours per week), part time hours would be considered at a minimum of 28 hours per week.

Responsibilities

* Deliver polypharmacy reviews, medicines reconciliation, and management of acute and repeat prescription requests.
* Conduct medication reviews and other work in line with the GMS contract.
* Collaborate with GP Practices as part of a multi-professional team to optimise safe, high-quality, evidence-based and cost-effective use of medicines.
* Work with an integrated pharmacy service and MDT colleagues to deliver high quality pharmaceutical care and ensure optimal patient outcomes.

Development and Environment

As an integrated pharmacy service, we run regular staff development sessions on a range of clinical topics, and, along with our MDT colleagues, there is regular opportunity to attend wider educational and CPD events. This role provides a developmental opportunity to gain breadth of experience across the diverse, interesting and engaging clinical nature of primary care. Although primary care experience is desirable, candidates from other sectors are encouraged to apply as full training will be provided.
