Job Description:
We are seeking motivated and adaptable Locum Band 5 Community Nurses to join our healthcare team for ad hoc shifts. As a Community Nurse, you will provide essential nursing care and support to patients in their homes and within the community. Your role will involve a variety of clinical tasks and responsibilities to meet the diverse healthcare needs of our patients.
Key Responsibilities:
Patient Assessment and Care:
* Conduct comprehensive assessments of patients in their homes to determine their healthcare needs.
* Administer prescribed medications and treatments, ensuring accurate documentation.
* Provide wound care, catheter care, and other clinical procedures as required.
* Monitor vital signs, observe for changes in condition, and promptly report any concerns to the appropriate healthcare professionals.
Health Promotion and Education:
* Educate patients and their families on managing their health conditions and promoting wellness.
* Offer guidance on medication management, dietary needs, and lifestyle adjustments.
* Provide support for patients with chronic illnesses to help improve their quality of life.
Medication Management:
* Prepare and administer medications, adhering to established protocols and safety guidelines.
* Ensure medication compliance and provide education on drug interactions and potential side effects.
Documentation and Reporting:
* Maintain accurate and up-to-date electronic health records (EHR) for each patient encounter.
* Complete incident reports and communicate patient status changes to the nursing team and healthcare providers.
Collaboration and Communication:
* Collaborate with multidisciplinary healthcare teams, including physicians, therapists, and social workers, to coordinate patient care.
* Communicate effectively with patients, families, and caregivers to address their concerns and questions.
Safe and Ethical Practice:
* Adhere to professional standards, ethical guidelines, and infection control measures.
* Ensure the safety and well-being of patients during all care interactions.
Ad Hoc Shifts:
* Be available for ad hoc shifts as needed, including weekends.
* Maintain flexibility in scheduling to accommodate patient needs and team requirements.
Qualifications and Requirements:
* Registered Nurse (RN) qualification with valid nursing license.
* Community Nursing experience.
* Current Basic Life Support (BLS) certification.
* Strong clinical assessment and critical thinking skills.
* Excellent communication and interpersonal abilities.
* Proficiency in electronic health record (EHR) systems.
* Compassionate and patient-centered approach to care.
* Ability to work independently and as part of a healthcare team.
* Reliable transportation and willingness to travel to patient homes within the community
