Responsibilities
* Deliver high quality, personalised, compassionate care to service users, providing a range of specialist interventions.
* Ensure consistently high standards of practice and clinical excellence, complying with the Nursing and Midwifery Council (NMC) code of professional conduct.
* Lead delivery of comprehensive assessment, planning, implementation and evaluation of patient‑centred care programmes in a range of settings.
* Provide clinical supervision and support to junior colleagues and students.
* Assist and support the manager to deliver a nurse‑led integrated care service and develop a specialist practice‑based clinical model of care.
* Maintain a safe and therapeutic working environment, working collaboratively with the multi‑disciplinary team and maintaining accurate records.
* Support children and young people with complex medical needs at home, in schools and in paediatric continence clinics within two boroughs.
Person Specification
Education
* NMC registered nurse with child nursing experience, or a registered child nurse.
* Evidence of CPD related to a specialist children’s nursing subject.
Experience
* Experience of working with children and young people in the community.
* Experience of working with children and young people with complex medical needs.
* Knowledge and understanding of child continuing care.
Knowledge
* An understanding of complex needs and the interventions that service users may require.
Skills
* Clinical skills in assessing a deteriorating child.
* Experience in tracheostomy, ventilation and seizure management.
