Head of Adult Community Team, Early Intervention & Prevention
Bracknell Forest Council is looking for a Head of Adult Community Team, Early Intervention & Prevention. The role requires a registered Social Worker, Occupational Therapist or Nurse. This permanent position offers flexible hours and a competitive salary of £68,505 - £74,122 per year inclusive of London weighting.
Key responsibilities
* Lead and manage a multidisciplinary team of health and social care practitioners.
* Deliver social care and CQC‑regulated reablement services for older people and adults with long‑term conditions.
* Support the Assistant Director of Adult Social Care Operations in formulating the strategic direction, priorities and plans for the service.
* Oversee the Hub; the Early Intervention and Prevention Team; the Reablement and Intermediate Care Team; the Hospital Team; the Adult Community Team; Sensory Needs and Blue Badges.
* Model the council’s values and behaviours – inclusive, ambitious and always learning.
About You
You have senior leadership experience and a strong commitment to delivering the CQC improvement plan, ensuring the adult social care service maintains its “Good” CQC rating while embedding the Target Operating Model. You possess effective budget management and performance data analysis skills, and you have experience managing multi‑disciplinary teams and collaborating with partners across the health and social care system.

The adult social care service is comprised of highly professional and supportive multidisciplinary teams operating under a “can do” and “no wrong door” ethos.
